Job Description

Chronic Care Management Registered Nurse

Location: Bohemia, NY

Hours: Monday- Friday

Organization: New York Cancer and Blood Specialists

In This Role, You Will

• Establish, implement, and revise patient and disease-centered care plans
• inventory the patient’s physical, mental, cognitive, psychosocial, functional, and environmental determinants of health
• Create comprehensive care plans based on the patient's health.
• lead LPNs and MAs in the implementation of a written plan of care.
• Utilize standing protocols to initiate therapeutic measures and call for crisis intervention when appropriate.
  
  

We Require

• Graduate from an accredited School of Nursing and an unencumbered New York license.
• Nursing Experience which one has acquired clinical competence
• Excellent assessment, communication and organizational skills.
• Good computer knowledge.
• The ability to plan, organize, develop, and interpret health data, goals, and objectives that are necessary for providing quality coordination of care.
• Bi-Lingual, English/Spanish a plus.
• HemOnc experience preferred.
